## **Steps to Cancel your Subscription:**

1.  Log or sign into into your Viome account at https://my.viome.com.
2.  After logging in, click "**Account**" icon and then "**Manage Subscription**." You will see an option to cancel your subscription.

3.  Note that for subscriptions that included a test in your monthly subscriptions fees, you may be redirected to contact customer service for further assistance.

**If a supplements order has been charged or billed, that supplements order cannot be cancelled or refunded** because all supplements orders are custom-made and cannot be used by anybody else. This applies even if your subscription has been cancelled. Supplements start manufacturing immediately after getting charged.

**Subscription Cancellation Fee Policy**

  
Subscription plans that "bundle" a Full Body Intelligence, Gut Intelligence, or Oral Health Intelligence test with monthly supplements and/or lozenges

*   **Full Body Health Solutions** — May also be known as the Full Body Essentials or Complete Plan is an annual subscription plan (billed monthly) that includes one Full Body Intelligence Test kit per year and monthly Precision Supplements (capsules) & Probiotics + Prebiotics (stick packs). If you choose to cancel this subscription within four (4) paid orders **OR** do not collect your samples and return your test kit within four (4) months of starting your subscription , you will be subject to a cancellation fee equal to one (1) month of subscription fees. This fee is charged to cover the full price of the test kit you received.

  

*   **Gut Health Solutions** — May also be known as Gut Health Essentials is an annual subscription plan (billed monthly) including a Gut Intelligence Test kit per year and monthly Probiotics + Prebiotics (stick packs). If you choose to cancel this subscription within four (4) paid orders **OR** do not collect your samples and return your test kit within four (4) months of starting your subscription , you will be subject to a cancellation fee equal to one (1) month of subscription fees. This fee is charged to cover the full price of the test kit you received.

  

*   **Oral Health Solutions** — May also be known as Oral Health Essentials Includes an Oral Health Intelligence Test + VRx MyBiotics Oral Lozenges and/or Precision Supplements is an annual subscription plan (billed monthly). If you choose to cancel this subscription within four (4) paid orders **OR** do not collect your samples and return your test kit within four (4) months of starting your subscription , you will be subject to a cancellation fee equal to one (1) month of subscription fees. This fee is charged to cover the full price of the test kit you received.

  

Subscription plans where supplements are purchased separately from a test kit (typically after test kit is purchase or after test results are released):

*   **Personalized Supplements (Precision Supplements™)** — A monthly subscription of supplements (capsules) that are purchased without a test kit included have no cancellation fees.

  

*   **Personalized Gut Biotics****( My·Biotics™ Gut Formula)** — A monthly subscription probiotics + prebiotics (stick packs) that are purchased without a test kit included have no cancellation fees.

  

*   **Personalized Oral Biotic Toothpaste (VRx My·Biotics™ Toothpaste & Gel)** — A monthly subscription of morning (AM) and evening (PM) toothpaste & gel that are purchased without a test kit included have no cancellation fees. Requires a Full Body Intelligence or Oral Intelligence test to be purchased separately.

  

*   **Personalized Oral Biotic Lozenges (VRx My·Biotics™ Oral Lozenges)** — A monthly subscription of daily lozenges that are purchased without a test kit included have no cancellation fees. Requires a Full Body Intelligence or Oral Intelligence test to be purchased separately.
