Important: Before shipping your sample(s), make sure your test kit has been registered and your Questionnaire has been completed. Samples cannot be processed by the lab until these steps are finished.

## **How to Send Your Test Sample(s) to Viome From Outside the United States**

1.  After collecting your sample(s), place the collection tube(s) into a secure shipping envelope.
    
2.  Insert the completed **Certificate of Non-Biological Material** form inside the envelope.
    
3.  Attach the completed **CN22 Customs Declaration** form to the outside of the envelope.
    
4.  Process your return here: [Returns Portal](https://returnsportal.shop/viome)
    
5.  Ship your sample(s) to Viome’s lab
    
6.  Keep your shipment tracking number for reference in case any delivery issues arise.
    

Please note that customers are responsible for all international shipping and customs-related costs associated with returning samples to Viome.

Your collected sample(s) remain stable for up to 4 weeks while in transit. If a sample arrives compromised or cannot be processed, Viome will provide a replacement test kit at no additional cost.

*   Attach this form to the outside of your shipping envelope.
    
*   Complete all required fields, including your signature and date.
    
*   Enter the appropriate tariff information in the section labeled **“For commercial items only.”**
